heparin (Heparin) (unfractionated)
Class:
Anticoagulants: IV/Subcutaneous Anticoagulants
Indication:
Prevention and treatment of thromboembolic disorders (e.g., stroke, DVT, PE).
Mode of Action:
Activates antithrombin, inhibiting thrombus formation. Prevents new clot formation. Does not break up existing clots.

Indication:
Bleeding, heparin-induced thrombocytopenia (HIT), Spinal/epidural hematomas (with spinal catheter).
NC/PT:
Closely monitor aPTT. Therapeutic aPTT is 1.5 - 2 times baseline. Antidote is protamine sulfate! Bleeding precautions (card 24).
