Indication:
Serious bacterial infections, antibiotic-associated C. diff.
Mode of Action:
Destroys bacterial cell wall, causing cell death.
Cautions:

Indication:
Ototoxicity, nephrotoxicity, infusion reactions (i.e., vancomycin flushing syndrome or "red man syndrome"), phlebitis.
NC/PT:
Monitor trough levels (and possibly peak levels). Dose may need to be adjusted depending on creatinine levels. If an infusion reaction occurs, decrease infusion rate per order. Antibiotic considerations (card 22).
